Health Quest for Chronic Disease Prevention

Health Quest for Chronic Disease Prevention Explained

This is an interdisciplinary and interactive research action project for students in middle grades 5-8. It will meet educational standards for health, math, writing, reading and communications.

Time Frame

This unit includes 5 Lesson Tasks and a Final Project. The entire unit will take 3-4 months to complete. Feel free to use one or all of these assignments in your classroom.

Unit Outcome

This unit will enable middle school students to identify and explain the impact of chronic diseases on people in the world, assess the risk factors for chronic diseases in their own community and introduce a plan for prevention to their school board or town leaders.

Teacher Expectations:

  1. Administer 5 lesson tasks that are designed to prepare students for their final project.
  2. Identify guest speakers to present about each topic and be available for student questions. (Suggestions for speakers are provided).
  3. Once students have completed all 5 assignments, teachers present them with a multi-faceted final project that involves surveying community members, interpreting survey results, creating an action plan based on results and presenting a plan to the school board or community leaders. All tools for this project are provided.

Please note that the Pre and Post Test results should be submitted to the Center for Health & Learning at 28 Vernon St Suite 319 Brattleboro VT 05301.

Student Expectations

  1. Complete 5 short research assignments using web resources, personal interviews and fact sheets.
  2. In assignment 5 students prepare a 5-minute presentation for their peers about chronic disease.
  3. Upon completion of all 5 assignments, students will be responsible to carry out final project that includes surveying community members, interpreting survey results, creating an action plan based on results, and presenting a plan to the school board or community leaders. All tools for this project are provided.
