Final Project GuidelinesBackgroundUp to this point all Learning Tasks have prepared students to face the challenge of this final project. For their final project, students will work together AS A CLASS to implement a survey that determines chronic disease risk factors in their school community. With this information students will prepare an action plan for disease prevention and will present it to their school board or community leaders. While the framework of tasks is laid out in this lesson plan, teachers will need to play a very supportive role in administering project guidelines, collecting data and preparing a presentation. Support materials are included to assist you and students. Phase 1: Administer Health Assessment Survey to Whole SchoolUsing the Chronic Disease Risk Assessment provided, students will survey a sample of the school community (e.g., two or more classrooms, staff, etc.). Phase 2: Interpret Survey ResultsOnce the surveys are complete, students will compile and tally the results in the Excel spreadsheet provided (this will need to be done on a computer). The Excel file will allow students to sort data by different categories and to analyze the information from different perspectives. From these results, students will identify the most relevant risk factors for their own community members. Excel Spreadsheet Directions for Spreadsheet Phase 3: Create an Action Plan Based on Survey ResultsOnce students have identified community risk factors, a student leader or teacher should help facilitate a classroom discussion to decide on some action steps that will reduce the risks of chronic disease. Use the action planning tool provided to help with this process. Phase 4: Present Survey Results and Action Plan to School BoardUsing your survey results and action steps, have students prepare a presentation for your community leaders (school board, PTO, School Health Action Committee, etc.) requesting resources or support for implementing their action plan. Phase 5: Documenting and Assessing your Health Quest ParticipationUpon completion of your project, please administer the Post Test provided (and submit to the Center for Health & Learning) and distribute certificates of participation to students. We also hope you will send us any documentation, pictures or success stories of your experience with Health Quest. We will post these items on our web site for future participants. |
